Using Galileo Risk Stratification to Manage a Population

Galileo Risk Stratification can be used to produce a worklist of stratified patients to organize proactive care management workflows.

The worklist should:

• be re-computed nightly
• show risk level assignment and the specific qualifying criteria that were applied to each patient to assign the risk level
• be presented along with other useful information to prioritize patients such as recent ED/Inpatient utilization or status of gaps of in care

The worklist can be summarized in an overview Risk Board as shown below:

For a given patient, we can see a trend of prior risk level and how it relates to overall utilization.
