Risk Stratification
Learn more about the Galileo Risk Stratification Framework (GRSF). GRSF computes historical risk factors in a vectorized model that can be used to relate current risk profiles to prospective utilization that is targeted by care management programs. GRSF performs whole-population risk stratification and assigns risk levels based on real-time risk factors detected from claim-sourced and EHR-sourced data for a population.
Key differentiators of our approach:
- Focus on high risk of emergent, unplanned utilization in the next 90 days. This requires up-to-date source data (favoring clinical sources) and daily enrichment cycles.
- Focus on high baseline risk concurrent with recent instability (ED, change in risk score, change in lab result).
- Focus on modifiable risk factors such as substance abuse, behavioral health changes. SDOH may not be predictive but can suggest a management approach.
